Minimum Qualifications:

Bachelor's degree in Psychology, Social Work or Mental Health / Behavioral Science.Criminal Justice degree considered with 15 hours of coursework in psychology, social work and/or counselingwith2 years of MH case management experience

Job Description:

This position is responsible for providing case management, social and habilitative services, and counseling to patients. Works under the supervision of a Psychologist, Mental Health Manager or Program Director.

Job Duties:

+ Triage calls received by UTMB’s CPAN team.

+ Fulfill resource requests.

+ Refer calls to providers for consultation.

+ Enroll clinics and providers in CPAN and PeriPAN.

+ Contribute to the resource database for all projects.

+ Liaisons with patient, patient families, on-site departments and community agencies to coordinate current treatment and/or assist with aftercare planning.

+ Create and develop mental health education materials as assigned.

+ Attend project related meetings and participate in project related activities.

+ Adheres to internal controls and reporting structure.

Marginal or Periodic Functions:

+ Performs related duties as required.

Salary Range:

+ Commensurate with experience.
