    St Croix Hospice is hiring an RN Case Manager for our branch in Brighton, MI.

    What will you do?

     

    TheRegistered Nurse(RN) Case Managerplans, organizes and directs hospice care with emphasis on community health education/experience. You will build from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

     

    + You will complete an initial, comprehensive, and ongoing assessment of patient and family to determine hospice needs.

    + You will complete, maintain, and submit accurate and relevant clinical notes regarding patient’s condition and care given.

    + Travel throughout the day and sometimes after-hours

    + Case Managers also work an on-call schedule on a rotating basis.

    Qualifications

    Required

    + RN licensure for the state(s) you work in

    + Clean driving record, and valid driver’s license and auto insurance

    + 1+ years of nursing experience in acute care, long term care, or hospice/home health

    + Hospice experience strongly preferred
