Job Title:** **Acute Transitional Care Nurse (RN) – Telecommuter
**Department:** Comprehensive Clinical Management
**Schedule:** Monday–Friday, 8:00 AM–5:00 PM EST
**Location:** Remote (Telecommute)
Pay: 43-45/hr
Position Overview:
The Acute Transitional Care Nurse (RN) plays a critical role in ensuring seamless transitions of care for patients across inpatient and outpatient settings. Operating within established contracts and health plan benefits, this nurse leverages advanced clinical knowledge to optimize care coordination, resource utilization, and patient outcomes. This role requires a proactive approach to continuity of care, working closely with primary care providers (PCPs) and interdisciplinary teams in a leadership capacity to achieve site-based goals.
Key Responsibilities:
+ Prioritize patient care needs during initial contact and respond promptly to emerging concerns.
+ Conduct meetings with patients, families, and caregivers to discuss treatment plans and care transitions.
+ Facilitate care transitions across acute care, skilled nursing, custodial, and ambulatory settings.
+ Utilize clinical pathways, protocols, and support resources to enhance outcomes and ensure continuity.
+ Manage discharge planning from facilities, including coordination of follow-up appointments, home health, social services, transportation, and more.
+ Maintain clear communication with physicians, hospitalists, extended care facilities, patients, and families.
+ Monitor and analyze the cost implications of care decisions using reporting tools and metrics.
+ Collaborate with physicians and care teams to implement and adjust patient care plans.
+ Support patient self-management through motivational interviewing and behavior change strategies.
+ Address advance care planning, including discussions on Durable Power of Attorney (DPOA) and advanced directives.
+ Educate patients and families on benefits, resources, and care plan elements.
+ Accurately document patient information in electronic health systems.
+ Provide remote and on-site clinic support as required.
+ Travel as needed to meet patients and families in accordance with care plans.
+ Perform additional duties as assigned.
Required Qualifications:
+ Bachelor’s degree in Nursing (or Associate’s degree with 4+ years of clinical experience).
+ Active and unrestricted RN license in the state of Florida.
+ Minimum of 2 years of clinical experience in a physician’s office, clinic, or hospital setting.
Preferred Qualifications:
+ Experience in care continuum management, utilization review, or discharge planning.
+ Previous HMO experience.
+ Certified Case Manager (CCM) certification is a plus.

**Pay Details:** $43.00 to $45.00 per hour
