Job Summary and Qualifications
The Occupational Therapist provides high-quality, patient-centered care by managing the complete process of occupational therapeutic care of an assigned group of patients, in coordination with the physician and the patient care team. The Occupational Therapist completes patient assessments, and participates in developing and implementing individualized care plans, focusing on goal achievement, patient engagement, and positive patient experience. The Occupational Therapist provides or directs a majority of the patient’s physical therapy to support patients throughout home health and the continuum of care.
Position summary:
This position is designed to provide occupational therapy services, including the evaluation of inpatients, outpatients and/or inpatient rehabilitation unit patients, development, coordination, and implementation of the treatment plan. Also provide maintenance of informative, accurate, and timely documentation; participation in team/family conferences, departmental/hospital committees; training of others as indicated; and participation in continuing education.

+ Adapts to change, is open to new ideas, volunteers for new responsibilities, and adjusts plans and is flexible enough to meet changing needs of the position
+ Expresses frustration assertively and professionally to the appropriate individual.
+ Evaluate patients
+ Performs OT screenings as ordered within 24 hrs. of date & time of referral
+ Perform initial evaluation within the specified time frame.
+ Perform objective evaluations properly according to the instructions.
+ Evaluates patient utilizing knowledge of the developmental needs appropriate to the age of the patient (adolescent, young adult, adult and geriatric patients)
+ Evaluates patient utilizing knowledge and skills of the diagnosis for each patient.
+ Perform follow-up evaluations as needed.
+ Re-evaluate the patient every two weeks or more frequently to adjust treatment plans as needed.
+ Perform home evaluations
+ Develop treatment plans and goals:
+ Establish measurable, functional, realistic long- and short-term goals, utilizing knowledge of the diagnosis and developmental need appropriate for the age of the patient (adolescent, young adult, adult and geriatric patients)
+ Develop treatment plan within the specified time frame.
+ Evidence that the patient has participated in establishing his/her treatment plan.
+ Adjust treatment plan as needed, clearly identifying which goals were met or not met and explain.
+ Implement Treatment Plan
+ Conduct appropriate treatment plan to include ADLs, and ADL/adaptive equipment training or fabrication, ROM, strengthening, cognitive/visual-perceptual, and/or sensory exercise. Select and utilize modalities and age-appropriate treatments that are indicated for the diagnosis or by the physician’s prescription. Demonstrate knowledge of the developmental needs appropriate to the age of the patient.
+ Communicate with the physician about changing the treatment program and/or suggesting treatment not originally prescribed by the physician. · Report significant findings by telephone to the physician(s).
+ Instruct, train, and provide consultation to patient, family members, and significant others.
+ Maintain a current prescription for all treatment.
+ Coordinate treatment with other team members.
+ Coordinate scheduling of patients.
+ Secure and maintain all necessary supplies and equipment.
+ Develop appropriate home exercise programs.
+ Fabricate appropriate splints for patients.
+ No more than five complaints received about the employee during the year.
+ Patients treated by the employee progress in their ability as documented in the patient’s chart.
+ No more than five negative comments received about the employee on that patient satisfaction questionnaire.
+ Comply with all department/hospital safety, infection control, and quality assurance policies and procedures.
+ Perform an average of 75% direct patient care productivity (6 hours or 24 units) within an 8-hour work hour day, (minus ½ hour lunch break) census permitting.
+ Flexes workday based on patient care load.
+ Initiates offers of assistance to fellow staff members or director, when schedule permits.
+ Completes Documentation
+ Complete evaluations within 24 hours, including the treatment plan (LTG & STG).
+ Complete daily progress notes on all patients by the end of the treatment day. Document home evaluations and recommendations within 24 hours of the visit.
+ Complete discharge summaries within 24 hours of discharge. Maintain daily patient assignment sheets and charges for each patient.
+ (Rehab Unit) Complete schedule board the day before, indicating which patients will be receiving ADLs in the morning, communicating this in writing to the patient.
+ Respect confidentiality/privacy of patient records and the patient during treatment.
+ Co-signs all Certified Occupational Therapy Assistant Documentation Participates in patient/family conferences & committees
+ Maintain assertive, effective communication with all team members.
+ Attend and contribute to patient/family conferences to discuss patient’s condition, progress, goals and plans.
+ Consult with physician(s) and other team members regarding specific requests and/or suggestions
+ Promote teamwork and cooperation within the department and hospital.
+ Volunteers to act as committee chairperson periodically.
+ Ensures committee meets monthly or more frequently, based on the project demands.
+ Writes monthly or quarterly report, representing committee progress.
+ Provide training/learning opportunities as assigned
+ Provide appropriate training/learning experiences for assigned professional staff, students, volunteers, technicians, tour groups, etc.
+ Provide input or write the evaluation for those assigned in a timely manner
+ Coordinate student program if assigned.
+ Provide marketing of services for the department, as directed.
+ Participate in continuing education opportunities
+ Attend 80% of department in-services/scheduled meetings
+ Maintain current CPR certification
+ Attend all mandatory in-services for department or hospital
+ Complete HealthStream education as assigned
+ Responsible to schedule & provide department/staff in-service after attending approved continuing education.
What qualifications you will need:
Essential skills and experience:
+ Master’s degree in occupational therapy from an accredited university
+ Indiana licensure through the Indiana Health Profession’s Bureau
+ 1-3 years of experience working in a healthcare setting, preferably a hospital
+ Certification in BLS must be maintained throughout employment with Terre Haute Regional Hospital
