    The Lead Outpatient Care Manager is responsible for assisting the Care Team in managing the workflow questions and case questions of their identified team members while still maintaining a current case load. Will help guide their identified team members in troubleshooting difficult cases around resources and case closure.

    JOB DUTIES AND RESPONSIBILITIES:

    + Provide real time case support for identified (resource review, problem solving, communication guidance, etc).

    + Provide workflow/documentation support.

    + Assist with chart audits as needed.

    + Assist with ‘go-live’ support as needed.

    + Participate in work groups or committees as assigned by supervisor or director.

    + Provide CHW escalation coverage if assigned Care Manager is not available.

    + Assists in orientation process.

    + Provide feedback to supervisor/director for staff meeting agenda items.

    + Maintains a full CM caseload.

    + Communicates with and coaches patients to ensure that they are aware of discharge instructions; have necessary prescriptions; have access to medications and understand how to take the necessary medications, including what to look for regarding adverse events as per their care givers’ instructions.

    PHYSICAL AND SENSORY REQUIREMENTS:

    Requires sitting, standing, and walking for extended periods of time (up to 8 hours at a time). Continual use of fingers for typing, writing and electronic handling equipment. Routine use of upper extremities. Occasional requirement to lIft items up to 25 pounds.

     

    Regularly requires ability to stoop, bend, reach above shoulder level, and climb stairs. Requires the ability to hear normal conversation and good general, near and peripheral vision.

    EDUCATION:

    Master's Degree in Social Work required. Registered Nurse (BSN preferred).

    TRAINING AND EXPERIENCE:

    Registered Nurse License required. BLS certification required. LSW preferred. CCM certification preferred. Training/experience in Motivational Interviewing and Teachback preferred. Ambulatory Care Management experience preferred. Bilingual preferred. Electronic Health record documentation experience preferred.
