    Job Description

    The case manager will provide an array of services to support adults, children and families, such as adult and child case management, skill building, service coordination and referral, and supportive counseling.

     

    The services are delivered in a variety of home, office, community-based, depending on the client’s needs, including the Behavioral Health Home model.

     

    Must have an active Maine driver’s license and reliable transportation
