Martha’s Hands is expanding our team of Nurse Care Managers for Private Pay Home Care Clients!

Martha's Hands Nurse Care Managers:

Utilize your Nursing Skills as part of a 12-person Care Supervisory Team to:

+ Guide a client and their family through their care needs.

+ Support/Train our Caregivers in providing direct care to our clients in their homes.

+ Partner with home health, hospice and other care professionals to manage their care.

Overview:

Private Pay Clients Only - No Medicare or Insurance Documentation!

Our Nurse Care Managers are compassionate, health care professionals who:

+ Conduct Assessments with clients and their families in order to develop a customized Service Plan.

+ Orientate and train our Caregivers to provide excellent care.

+ Provide clinical oversight of our clients and support to our caregivers.

+ Provide direct care to our clients - perform Skilled Nursing tasks and ADL assistance as needed, including Medication Management, Wound Care, Diabetic Care, Tube Feedings, etc.

+ Assist families with more detailed Care Management Services as needed.

+ Balance both an office and mobile work environment, utilizing technology to enhance the care for our clients and equip our Nurse Care Managers to be more efficient and effective.

+ Supervise care team to ensure care compliance, care modifications and quality/customer service while inspiring and encouraging our Caregivers.

Required Experience:

+ 5+ years nursing experience. Ideal experience in Private Duty, Home Health, Hospice, Geriatric Nursing or Med Surg.

+ Team Approach to care by utilizing strong professional, interpersonal communication skills.

+ Strong clinical assessment skills to develop a customized service plan that directs the care for our clients.

+ Educational and training abilities to enhance Caregiver’s success in caring for our clients.
