    *_Position Summary _*

     

    Investigates, assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems.

    _*Minimum Qualifications* _

    * *Minimum Education**:* MSW preferred or BSW required.

    * *Minimum Experience**: * Must possess familiarity with case management issues, discharge planning and utilization management.

    * *Licensure/Certification:* NM Social Work license or Provisional (PLBSW). While provisional or license is being obtained, will work under the NM license and supervision of a current Licensed Social Worker or Registered Nurse for up to 6 months.
