    Requirements

    Key Responsibilities:

    + Conduct ongoing clinical assessments, including OASIS assessments, to identify patient/client needs and determine levels of functioning.

    + Evaluate patients’ occupational performance in areas such as self-care, work, leisure, and rest.

    + Assess sensory-motor neuromuscular, psycho-social, and cognitive-perceptual components of occupational behavior.

    + Evaluate physical, social, and cultural components of patients’ occupational environment.

    + Facilitate direct therapeutic interventions to address ROM, strength, coordination, adaptive equipment needs, and environmental modifications.

    + Develop, evaluate, and revise the plan of care in collaboration with patients, representatives, and caregivers.

    + Document clinical notes, phone orders, and other records in a timely manner.

    + Accept clinical assignments consistent with education and competence.

    + Provide physician-ordered services, including patient/client, caregiver, and family counseling and education.

    + Supervise and guide occupational therapy assistants and home health aides as appropriate.

    + Apply infection control and standard precautions to prevent contamination and transmission of disease.

    + Utilize effective interpersonal relations and communication skills to achieve desirable outcomes.

    Job Qualifications:

    + Education:Bachelor of Science in Occupational Therapy.

    + Licensure:Current State License as an Occupational Therapist, Current driver’s license required.

    + Experience:One year of experience as an Occupational Therapist preferred. Home health experience preferred

    + EMR:Home care home base proficiency preferred

    + Skills:Proficiency in therapy skills as per accepted standards of practice, good interpersonal skills, current CPR certification, and knowledge of durable medical equipment.
