Private Duty LPN – Full-Time – Greenfield, TN

Overview:

Join our Adoration Private Duty team and experience the difference of one-on-one in-home care. We’re currently seeking a compassionate and skilled LPN for full-time night shifts in Greenfield, TN. This role is perfect for nurses who value personal connections and want to make a real impact with a consistent patient.

Highlights:

+ 3/4 split schedule

+ 11-hour night shifts: 7:00 am - 6:00 pm

+ Full-time hours (84 hours every two weeks)

Patient Needs:

+ G-Tube care

+ Seizure monitoring and precautions

+ Total care needs in a home setting

Responsibilities

+ Perform selected nursing procedures within the scope of LPN practice

+ Provide patient and family teaching according to plan outlined by the RN

+ Monitor patient's vital signs and make specific measurements and observations

+ Monitor and record intake and output. Report significant changes immediately

+ Observe, assess and record patient's physical and mental condition and reports any changes

+ Administer medications as instructed and as ordered by the physician and instructed by the RN and document the patient's file accordingly

+ Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned

Qualifications

+ Licensed Practical Nurse / LPN license in good standing

+ Previous private duty, home health, hospice, hospital or skilled nursing experience preferred

+ Active CPR certification

+ Familiarity with electronic health records

+ Strong problem solving and critical thinking skills

+ Reliable transportation, valid driver's license and current automobile insurance
