We are currently seeking a*Home Health Program Manager*to lead our Home Health department.
_SUMMARY_
Thispositionwillassumeoverallresponsibilityforoversightand managementofclinicalandoperational services provided by the department of Home Health including core and non-core services.//
*_RESPONSIBILITIES_*
* Assumesoverallresponsibilityforthedeliveryofcareprovidedbythestaffofthehomehealthprogram
* DemonstratesaworkingknowledgeoftheCMSregulations,theConditionsofParticipation, Joint Commission standards and the community standard of provision for home health care services and can exemplify that by the level of service provision within the home health program
* Understands the need to stay current with changes to rules, regulations, standards, and industrytrendsastheyrelatetotheclinicalpracticeandtheoperationalmodelofthedeliveryof home health care. (OASIS, 485 content and coding)
* Assumesoverallresponsibilityforoversightandmanagementoftheclinicalservicesprovidedby the clinical and non-clinical teams
* Understandstheneedtomaintainthenecessarystaffresourcesandknowledgetokeeptheorder management process current and in compliance with Hennepin Healthcare policy, Joint Commission, and CMS regulations
* Responsiblefordevelopingasystem ofoperationandcommunicationthatwillindicatea need for a proactive management plan to maintain the referral flow from the primary referralsources
* Demonstrates the ability to collaborate and work with the care providers on the multidisciplinary team inamannerthatenhancestheirabilitytoprovideahighstandardofexcellenceofserviceto all home health program clients
* Maintains a model of supervision with the team leaders responsible for the care provision that enhancestheirabilitytogrowprofessionallyandtoconsistentlyimprovetheirskills asmentorsto the members of the clinical staff
* Assumesresponsibilityfordevelopmentofclinicalpolicyandprocedure
* Maintains proactive role with team leaders in assessing the skill level, the staffing model, the caseloadmixandtheneedtointerviewandhirestaffinamannerthatallowsforcontinuityofcareprovision
* Understands the need for the development and ongoing management of a robust quality improvementmodelthatrelatesdirectlytothedeliveryofcare,costmanagement,andthevarious regulatory requirements noted above

* Servesasa"subjectmatterresource/person”fortheteam leadersandclinicalstafftoensurethat the delivery of care remains in sync with the plan of care that is documented within Oasis
* Conductsregularlyscheduledmeetingswithclinicalstaff
* Participatesinthedevelopmentofeducationforclinicalstafftocloseknowledgegapsandfacilitate excellence in clinical service delivery including, but not limited to, the development and performanceof annualcompetencytestingthat willenhancetheir performanceas careproviders
