Coordinates discharge plan for patients through collaboration with the multidisciplinary patient care team as well as external health care organizations and professionals for the provision of care before, during, and after discharge or transfer when applicable. Provides comprehensive care for patients in accordance with regulatory requirements and hospital policy. This may include all age groups as applicable. Graduate of an accredited School or College of Nursing and current licensure to practice as a Registered Nurse in the State of Rhode Island or in another state that participates in the Nurse Licensure Compact. or Bachelor of Arts or Science Degree in Social Work required. BLS from AHA and minimum 1 year acute care nursing experience required. BSN preferred. For AD graduates applying, must provide evidence of enrollment for BSN program with completion within 3 years of taking new position.
