The primary role of the Transplant Coordinator is to coordinate the care of patients seeking candidacy and those in receipt of solid organ(s) (heart, lung, heart-lung, liver, kidney, pancreas, vascularized composite allograft and multi-organ) and bone marrow for transplant. In collaboration with members of a multidisciplinary team, the Transplant Coordinator will enhance patient care by developing relationships through education and service. The Transplant Coordinator will serve as an integral member with a goal to deliver exceptional care to a specific population of patients experiencing end-organ failure or disease.
A caregiver in this position works days from 8:00AM - 5:00PM, with on-call requirements.
Responsibilities:
+ Presents the potential transplant candidate or the transplant recipient to the transplant physician and documents the need for additional assessment or intervention as determined by the care team.
+ Provides education to patients and caregivers with specific focus on a single phase of the transplant process to provide optimal outcomes following transplant.
+ Facilitates the scheduling of studies or procedures utilizing fiscal responsibility for patient and hospital costs.
+ Communicates with the referring physicians on the plan of care established for the patient in collaboration with surgical and medical teams.
+ Completes required documentation according to Organ Procurement Transplant Network (OPTN) guidelines including listing status changes and post-transplant follow-up information as applicable.
+ Reviews and collates medical information received from referring physician for patients seeking or undergoing transplant evaluation.
+ Transfers communication concerning transplant patient for peri-transplant evaluation and post-transplant care for physician and patient/caregiver and/or payors seeking information.
+ Coordinates all activities related to organ acquisition including evaluation of the patient workup information and/or post-transplant care.
+ Is available by pager and by phone to provide immediate assessment and delivery of care in a timely and cost-effective manner.
+ Other duties as assigned.
Education:
+ Completion of Registered Nurse program and must obtain Bachelor' Degree in Nursing (BSN) within three years of hire date as per Cleveland Clinic policy.
+ If hired prior to 2024, bachelor's degree in nutrition, dietetics or a closely related field.
Languages:
+ English
Certifications:
+ Current state licensure as a Registered Nurse (RN) or if hired prior to 2024, current state licensure as a Registered Dietitian (RD).
+ Basic Life Support (BLS) Certification
Complexity of Work:
+ Within scope of job, requires critical thinking skills, decisive judgement, and the ability to work with minimal supervision.
+ Must be able to work in a stressful environment.
Work Experience:
+ One year of experience in acute care, medical/surgical, or ambulatory setting within a hospital environment.
Physical Requirements:
+ Requires full range of body motion, manual finger dexterity and eye-hand coordination.
+ Requires working under stressful conditions and working irregular hours.
+ Must be willing to be on-call 24 hours a day, 7 days a week.
Personal Protective Equipment:
+ Follows standard precautions using personal protective equipment as required.
