Description:
The Population Health Pharmacist is responsible for leadership in the innovation, development, and implementation of primary care pharmacy management services across the MercyOne Iowa market. The Population Health Pharmacist acts as a liaison between patient, provider, and care management team members to promote appropriate and affordable medication use and oversees pharmacy quality measures related CMS Part D STARS and associated HEDIS measures. The population health pharmacist helps identify and address medication related healthcare quality issues and serves as a subject matter expert for the organization while supporting core clinical programs for the organization. This position will spend approximately 70% of the time on direct patient care (telephonically, virtually or in-person) and 30% on strategic work around optimizing pharmacy quality measures.
Core Responsibilities:
+ Researches and synthesizes detailed clinical data related to medications to assist providers and patients in achieving cost and quality goals.
+ Serves as a clinical resource for care management team, colleagues and practitioners for medication-related therapeutic interventions. Including disease specific management of the key drivers of total cost of care e.g. Diabetes, CHF, CKD, COPD etc. Including but not limited to the Genesis pateints
+ Evaluates pharmacy quality program data and works with stakeholders to identify, prioritize, and implement strategies to optimize medication use and manage pharmacy spending effectively.
+ Acts as a subject matter expert in pharmacy-related clinical measures for value-based care agreements during practice-level discussions.
+ Works to improve pharmacy quality by staying up to date on STAR and HEDIS measures and provides direct patient support to improve medication use quality
+ Creates and implements provider (and patient) education and resources to support appropriate medication use, improve clinical pharmacy measures and reduce pharmacy costs.
+ Builds impactful relationships with providers to close the feedback loop between pharmacy and provider.
+ Works collaboratively with PHSO care management executive team and leadership to achieve desired outcomes
+ Oversees medication affordability and access programs for the PHSO care management department
+ Address clinical questions and issues escalated from PHSO Medication Access Coordinator and/or PHSO pharmacy technicians.
+ Completes outreach to patients and recommends personalized strategies to improve medication adherence, affordability, and clinical appropriateness of care.
+ Creates and documents evidence-based recommendations to providers to optimize medication therapy and affordability/medication access.
+ Educates providers, nurses, and other care team members on cost effective and clinically appropriate medication use and influences payer specific formulary changes to impact prescribing trends
+ Engages with the MercyOne PHSO Chief Medical Office and Clinically Integrated Network Executive Directors for program design to optimize PHSO performance under value-based contracts
+ Collaborates with PHSO analytics to measure to evaluate the effectiveness of PHSO medication related programs
+ Maintains updated knowledge of legislative, regulatory, and reimbursement issues related to the practice of pharmacy in the state of Iowa
+ Represents MercyOne PHSO in payer, consortium and employer meetings as needed
+ Prepares presentations as assigned to leadership on specific pharmacy initiatives
+ Other duties as assigned
Qualifications and Requirements:
+ Pharmacy degree (BS or Pharm.D.) from an ACPE approved college of pharmacy required
+ Licensed or eligible within 180 days of hire for licensure pharmacist in Iowa
+ Board certification and/or residency training is preferred Previous integrated healthcare system, health plan, or PHSO is preferred
+ Minimum of 2 or more years of experience in medication therapy management, Medicare STARS or HEDIS quality measures, managed care pharmacy, or population health
+ Familiarity with CMS STARS and HEDIS measures Ability to analyze and manage pharmacy utilization data from EMR, payer data and other sources to define pharmacy management opportunities.
+ Understanding and experience with quality data, risk adjustment, value-based payment models Proven ability to innovate and learn new concepts quickly and then adapt from those concepts to develop and execute operational plans. Demonstrated inter-personal skills of collaboration, compassion, empathy, and ability to achieve objectives with others Detail orientated and strong follow-up skills.
+ Strong presentation and communication skills. Demonstrated ability to work with an executive team and providers on strategic and tactical activities.
+ Proven ability to collaborate and work effectively across a large organization or multiple organizations handling a variety of issues, tasks and thrives in an environment with competing priorities Background in strategy, program project management/process improvement, or business development
