    Position Overview:

    Provides consultation to clinical team members, direct education and consultation to patients and families. Fosters clinical integration with interdisciplinary team members by promoting and monitoring adherence to evidence-based practice recommendations, professional development and dynamic team interactions about improving care processes to optimize outcomes. Promotes a learning and safe environment using strategies to positively influence patient care practices and patient behaviors to improve compliance and understanding of diabetes as well as potential complications from disease when not well managed. Develops relationships with other Sutter Health entities, system-wide teams, professional societies and external specialty programs to ensure alignment with evidence-based practice for diabetes management.

     

    **Job Description** :

    EDUCATION:

    + Bachelor's: Health related field or equivalent education/experience

    + OR Other: Graduate of an accredited school of nursing.

    CERTIFICATION & LICENSURE:

    + RN-Registered Nurse of California

    + **OR** RD-Registered Dietitian

    + CDCES-Certified Diabetes Care and Education Specialist

    TYPICAL EXPERIENCE:

    8 years recent relevant experience

    SKILLS AND KNOWLEDGE:

    + Expert knowledge of diabetes education programs and the advanced clinical management of patients living with diabetes across the care continuum.

    + Education materials development with writing skills.

    + Proficiency in data and analytics with spreadsheets and analysis from large datasets from the EHR; email, navigating health system style programs for patient care.

    + Demonstrates ability to work/interact with a diverse population while respecting cultural and linguistic differences and fostering an inclusive work environment.

    + Verbal communication and interpersonal skills.

    Pay Range is $66.99 to $100.49 / hour

Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs.
