    GENERAL RESPONSIBILITIES:

    The Director of Health Home Care Coordination serves as a member of the Behavioral Health leadership team and is responsible for the oversight and provision of case management across the WYH network of care. Central to this role is the continued development, creation and transformation of systems of care that foster seamless transitions for patients in need of care coordination across multiple locations and settings. The Director will oversee the implementation of new initiatives, outreach, education, and community-based embedment(s)-designed to maximize referrals while increasing access to integrated care for those in need of care coordination services.

    SPECIFIC RESPONSIBILITIES:

    • Provides oversight for the Health Home Care Coordination program; maintaining compliance in operations and procedures among all program(s) including; Adult/HARPs, Specialty Mental Health/HH+ and AOT, and Children (SED)

    • Provides administrative oversight and supervision for program managers

    • Serves as liaison and fosters collaborative relationships between primary care providers and community partners

    • Facilitates the development, training and on-boarding for care coordination staff

    • Establishes a work environment to support member/family-centered services in response to emerging needs

    • Initiates care coordination service embedment(s) across agency departments, service lines, and locations

    • Maintains positive relationships with members, referral sources and community-based partners

    • Possesses a working knowledge of the electronic platform(s) adopted by the Lead Health Home(s) and other DOH reporting systems, and serves as a super-user for all software, for program documentation and reporting purposes.

    • Possesses an understanding of the important role of care coordination as it applies to integrated health systems and quality improvement within the patient-centered medical home model of care.

    • Displays an ability to plan, organize, and prioritize multiple activities, while maintaining positive and collaborative relations with WYH staff and external community partners.

    • Participates in the development and maintenance of necessary documentation and reports according to program regulations including the submission of monthly capacity and productivity reports.

    • Designs referral(s) and admission workflows to ensure productivity standards for care coordination programs;

    • Interfaces with Managed Care Organizations (MCO) Complex Care Managers as to ensure timely linkage to health home services for patients qualifying for Health and Recovery Program Services (HARPS).

    • Serves on the lead Health Home Steering and Operations Committee meetings, Health Home Sub-Committee meetings and community meetings, as assigned.

    • Gathers data and creates trending reports/narratives in order to inform process improvements, achieve outcomes and influence programmatic decisions.

    • Disseminates patient lists/Facilitates MAPP sessions for referrals supplied by the Lead agency, community based organizations and WYH referrals.

    • Completes, Monitors and Audits documentation as to ensure billing in accordance with Medicaid/DOH requirements.

    • Ensures compliance with local, state and federal regulation (i.e., Joint Commission, NCQA, NYSDOH, Lead Health Homes)

    • Adheres to National Patient Safety Goals as defined by NCQA and Whitney M. Young Jr. Health Services.

     

    Completes other duties, as assigned.

    Requirements

    MINIMUM QUALIFICATION:

    Bachelor’s Degree in a qualifying field with at least three (3) years of experience serving those with Severe Mental Illness (SMI), Alcohol/Substance Use (SUD), Severe Emotional Disturbances (SED), or Intellectual and Development Disability (I/DD) populations and a minimum of (2) years of related experience providing supervision and/or program oversight.

    PREFERED QUALIFICATION:

    Master’s Level Healthcare Professional with at least two (2) years of experience in a behavioral health or related human services setting providing supervision and/or program oversight.

    Salary range: $74,000 - $85,000 annually
