    The Ambulatory Telehealth RN is responsible for providing comprehensive nursing care to provide care coordination in the evaluation for potential kidney transplant patients. This position involves patient assessment, triage, care coordination, patient education, population health management, and patient navigation in UC Health ambulatory clinics using telehealth technology. The Ambulatory Telehealth RN plays an integral role in the management of patients’ care and is expected to collaborate with UC Health providers and staff to ensure high quality patient centered care.

     

    Pre-Kidney transplant coordinator

     

    Facilitates evaluation and care coordinator for potential kidney transplant patients.

     

    Assess patients with members of the multidisciplinary team to determine candidacy for transplant.
