VTE NP Duties
The purpose of the venous thromboembolism (VTE) Nurse Practitioner role is to ensure high quality VTE prevention care is embedded throughout the organization with the aim of reducing preventable patient harm from hospital associated thrombosis. The VTE NP will be an integral part of the multi-disciplinary thrombosis team and will take responsibility for leading VTE prevention care, and ensuring adherence to the VTE guidance, to include surveillance, analysis and improvement of VTE risk assessment rates, hospital associated thrombosis cases, VTE staff education, VTE patient information and appropriate use of thromboprophylaxis measures. Activity will involve data collection, analysis, presentation and dissemination, research, staff education activities, awareness initiatives and quality improvement work.
As a leading university medical center in the field, the VTE NP will support nursing and medical teams in providing leadership and maintaining VTE prevention as an important institutional patient safety and quality initiative.
As an integral part of the hospital VTE team, the NP will participate in associated thrombosis activities and initiatives via Medication Safety Sub-Committee for anticoagulation and thrombolytic issues, and PERT team.
The NP will require computer skills to navigate programs such as Microsoft Excel, Access, Word, Powerpoint, and RedCap.
General NP Duties
+ In collaboration with a licensed physician, provides health care to patients including diagnosis and treatment.
+ Obtains medical histories, performs physical examinations, makes diagnosis and orders appropriate and standard laboratory and diagnostic procedures as necessary.
+ Evaluates procedure results and finding to physicians.
+ Provides continuity of care by developing and implementing patients’ plans of care.
+ Provides patient education about underlying medical condition, self-management of acute or chronic illnesses and tailors instructions to patients individual circumstances. Instructs and counsels patients regarding therapeutic regimens, promoting wellness and health maintenance.
+ Prescribes medications.
+ Administers medication/immunizations, manages wounds and infections, etc.
+ Documents exam and progress notes.
+ In addition to office visits, the Nurse Practitioner performs other tests and procedures specific to the specialty.
+ Collects, monitors and reports on patient outcomes.
+ Functions independently to perform age-appropriate history and physical for complex acute, critical, and chronically ill patients.
+ Orders and interprets diagnostic and therapeutic tests relative to patients age-specific needs.
+ Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities.
+ Implements interventions to support the patient to regain or maintain physiologic stability, including but not limited to serving in the first assisting role.
+ Facilitates the patient s transition within and between health care settings, e.g. admitting, transferring and discharging patients.
+ Collaborates with multidisciplinary team members by making appropriate referrals.
+ Facilitates staff, patient and family decision making by providing educational tools.
+ Other duties as assigned by supervising physicians.
+ Ensures HIPAA guidelines are followed at all times.
Qualifications
REQUIRED QUALIFICATIONS:
BLS, ACLS & LMS training/attestation. NYS RN license. NYS NP license. NP certification. Possesses specialized knowledge and skills in VTE, including diagnosis, management, and treatment of both venous thromboembolism and its complications.
PREFERRED QUALIFICATIONS:
Strong leadership skills and experience.
