    Salary: $2619 / Week

     

    Amergis Healthcare Staffing is partnered with a much a respected facility in Haverhill, MA who is currently seeking a qualified RN Case Manager/Discharge Planner to assist them on a contractual basis.

    Details of the position include:

    Position: RN Case Manager/Discharge Planner

    Duration:

    • 8 weeks to start

    • Option for ongoing duration

    • Temp to perm option available – discussed directly with facility

    Schedule:

    • 40 hours per week

    • Monday – Friday

    • 8:30a – 5:00p

    Contract Requirements:

    • Minimum 3 years of experience as an RN

    • Recent or current case management experience in a rehabilitation setting

    • Familiar with discharge planning and Medicare guidelines

    • Familiar with case management standards/guidelines

    • EMR experience and familiarity
