Learning Support Transition Specialist JOB SUMMARY:The Learning Support Transition Specialist is responsible to assist with the facilitation of smooth transitions from the traditional school setting, due to juvenile charges, court assignment or other needs, to juvenile care facilities and upon release from one of these programs, facilitate the student's transition back to the appropriate school within the CRCSD. Facilitate the delivery of services in an easily accessible and supportive manner. Connect youth and families to social service providers, facilitates collaborative planning, and ensures that coordinated services are provided which match assessed needs.SALARY: $61,800 ORGANIZATIONAL AND REPORTING RELATIONSHIPS:The Learning Support Transition Specialist will report directly to the building administrator and will have a working relationship with the Learning Supports department as well as partnership and high collaboration with Juvenile Court Services. ESSENTIAL JOB FUNCTIONS:The essential functions as shown below represent only the key areas of responsibility; specific position requirements will vary depending on need. The primary population for the position is middle and high school youth who are under the jurisdiction of juvenile court and who are transitioning into the CRCSD from out of home placement facilities and youth who are leaving the CRCSD due to out of home placements. A secondary population for services is CRCSD youth identified by JCS as at-risk and in need of school-based services.
+ Services will typically be provided from 8:00 a.m. to 4:00 p.m. during the normal school year calendar and at attendance centers where CRCSD student(s) are being served. Staff may have duties that require meetings outside of attendance centers.
+ Assist with on-boarding of JCS youth at the beginning of the school year which may include locating families, registration including but not limited to appropriate tracking of school credits, transportation, school supplies and addressing other pertinent needs.
+ Provide school-based services to students in group and/or individual settings.
+ Provide support with student & personal skills, problem solving, accountability and acceptance of responsibility.
+ Assist with transitioning youth from residential settings to the school setting. This would include meeting with the youth in the school setting within 30 days prior to an out-of-home placement or meeting with the youth at the placement facility within 30 days of being discharged from that facility. Attend Individual Education Plan (IEP) meetings at placement facilities for identified youth.
+ Assist staff with strategies to support and meet students needs in the areas of behavior, truancy, and achievement of academic goals. Attend weekly Learning Support meetings as appropriate.
+ Connect youth and family to resources in the school and community.
+ Promote family engagement in problem solving for truancy and academic goals and school-based activities.
+ Provide active communication and develop positive relationships with parents, juvenile court, students, staff, and community members.
+ Attend home visits as needed or as assigned.
+ Attend meetings at the Juvenile Court Office at a minimum of a biweekly basis. Provide the JCO with information about what is going on with their JCS clients in school and make an assessment of what level of JCSL involvement is needed for each youth.
+ Participate in the discipline review process of JCS youth at the direction of assigned JCO or supervisor and support the youth’s return to the educational setting.
+ Maintain appropriate records and protect the confidentiality of data.
+ Attend the JCS juvenile court school liaison conference when held.
+ Submit monthly reports via email to the JCS Contract Administrator by the 20th of the following month. The monthly report shall include the name of the youth, referral source, school, and placement facility, date of placement and discharge, and status notes.
+ Act as a liaison between a youths home, school and the placement facility. They
shall collect grades, set educational goals, monitor and advocate for appropriate educational credits, and check status of educational enrollment while the youth is in the placement facility.
+ The position shall act as a liaison between the youth’s homeschool and the detention center. They shall check the online class progress and monitor and advocate any credits they have earned and provide proper communication with the homeschool to ensure the best outcome for re-entering classes.
+ The position shall assist youth with college entrance and enrollment preparation for youth returning to the community from out of home placements. They may arrange for college visits for youth if appropriate.
+ Monthly visits to court ordered placements to relationship build, check student progress, attend staffings

The position requires the commitment of professional working hours that may require the employee to extend beyond a typical 7:30 a.m. to 3:30 p.m. workday. The employee will frequently work in different areas of the classroom including in/at desks, on or near the floor, standing, in movement while supervising indoor and outdoor activities, etc. The employee will usually work in indoor temperatures but will work outdoors or when supervising students outside of the classroom.
