UMC is currently seeking an LPN: Licensed Practical Nurse for our Pitman Manor Community.
Pitman Manor , a UMC Community in Gloucester County, our senior community in Pitman New Jersey, is truly integrated into the life of the town. We are part of a beautiful residential neighborhood, near to highways and public transportation with easy access to both Philadelphia as well as the Jersey Shore.
The LPN: Licensed Practical Nurse will be responsible t o provide ongoing monitoring of residents' physical and mental status to assure maintenance of optional level of function. Responsible for the daily clinical aspect of the Resident Care Department, functioning to meet all residents' needs.
Requireme nts for an LPN: Licensed Practical Nurse :
+ High School diploma or GED.
+ Some nursing experience in a related field preferred.
+ Current New Jersey LPN: Licensed Practical Nurse License.
+ IV & CPR Certification preferred. IV certification must be completed within 6 months of hire.
Responsibilities for an LPN: Licensed Practical Nurse:
+ Participates in collection of data and needed assessments from the resident or other appropriate source(s).
+ Obtains report from the nurse he/she is relieving, provides report to the nurse coming on duty.
+ Attend and participate in all scheduled training, educational and orientation classes, programs and activities including annual mandatory classes.
+ Chart nurses' notes in an informative and descriptive manner.
+ Contributes to the resident's assessment (MDS/RAP's) and the development of a plan of care.
+ Complete accident/incident reports as necessary.
+ Admit, transfer, and discharge residents. Complete required documentation and notifications as necessary.
+ Administers (within UMC set time periods), and documents administration of medications, enteral nutrition and treatments per the physician's order and accurately records all care provided in the EHR. Adheres to the 6 rights of medication administration. Accountable for all narcotics and medication cart keys on the shift.
+ Collaborates with the resident, partners of healthcare and, when appropriate, the resident's responsible party in the interest of the resident's health and well-being.
