About the Role
We’re seeking a passionate and skilled **Occupational Therapist** to join our Home Health team. In this role, you’ll provide in-home evaluation and therapy for clients with physical, emotional, or cognitive limitations. You’ll help patients achieve greater independence through customized care plans, family education, and goal-oriented therapy interventions.
What You’ll Do
+ Conduct home-based assessments within 72 hours of referral (unless otherwise specified).
+ Design and implement individualized treatment plans focused on improving daily living skills and function.
+ Educate patients and caregivers on therapy techniques, home exercise programs, and insurance guidelines.
+ Participate in interdisciplinary case conferences and collaborate across care teams.
+ Maintain clear, accurate documentation in compliance with all home health guidelines and regulations.
+ Ensure care is age-appropriate across the lifespan, from infants to geriatrics.
What You’ll Need
+ **Bachelor’s degree in Occupational Therapy required** (Master’s preferred).
+ **Current Iowa Occupational Therapy License** (provisional license with supervision accepted for new grads).
+ **Valid Iowa driver’s license** , proof of insurance, and reliable transportation.
+ BLS certification and Mandatory Reporter certification.
+ 0–2 years of experience in occupational therapy (home health experience preferred).
+ Strong communication, organization, and independent problem-solving skills.
