RESPONSIBILITIES

I. JOB SUMMARY/RESPONSIBILITIES:
• Works with the Medication Safety Coordinator in planning, coordinating, implementing and evaluating activities involving medication safety throughout The Queen’s Health Systems (QHS). Leads medication safety assessments through Failure Mode Effects Analysis, Root or Apparent Cause analyses, educates providers and staff on the system-based causes for medication errors, communicates best practices regarding effective medication safety strategies to others, and interfaces with hospital and system pharmacy and quality representatives on medication safety issues.
• Generates and reviews data/metrics, analyzes medication events (i.e. medication errors, adverse drug events, workflows/processes), and participates in root cause analysis involving medication events.
• Oversees and leads QHS smart infusion pump drug error reduction software (DERS) across QHS, including drug reviews, edits and updates.
• Reviews and analyzes medication safety reports or applications for performance improvement opportunities.
II. TYPICAL PHYSICAL DEMANDS:
• Seeing, hearing, speaking, standing, sitting, walking.
• Frequent: reaching below shoulder level.
• Occasional: stooping/bending, kneeling, climbing stairs, walking on uneven ground, squatting, twisting body, finger dexterity, lifting usual weight of 5 pounds up to 25 pounds, pushing/pulling usual weight of 1 pound of force up to 5 pounds of force, carrying usual weight of 5 pounds up to 20 pounds, reaching above and at shoulder level, repetitive arm/hand motions, frequent gripping of an object.
III. TYPICAL WORKING CONDITIONS:
• Not substantially subjected to adverse environmental conditions.
• Exposure to hazardous drugs, antibiotics, chemicals, dust, odors and cleaning/ disinfectant fumes.
IV. MINIMUM QUALIFICATIONS:
A. EDUCATION/CERTIFICATION AND LICENSURE:
• Current Hawaii State license as a Pharmacist.
• Doctor of Pharmacy degree.
• Successful completion of the Medication Safety Certificate training program from the American Society of Hospital Pharmacy (ASHP). If course has not been completed upon hire, must be completed within six (6) months of entrance into position.
• Completion of a PGY1 or PGY2 residency/fellowship in Medication-Use Safety and Policy preferred.
B. EXPERIENCE:
• Three (3) years experience as a hospital Pharmacist; or completion of an American Society of Hospital Pharmacy (ASHP) PGY1 or PGY2 residency in Medication-Use Safety and Policy or Health System Pharmacy Administration and Leadership may be substituted for experience requirement.
• Prior performance improvement or process improvement methodologies, such as Six Sigma and Lean experience preferred.
• Experience to demonstrate:
o Knowledge of medication use and safety regulatory standards (The Joint Commission, CMS, Department of Health, Hawaii Board of Pharmacy, OSHA, United States Pharmacopeia, FDA).
o Knowledge of Microsoft Office applications (i.e. Outlook, Word, Excel, PowerPoint).
o Effective written and oral communication skills and interpersonal skills.
o Excellent organizational skills and ability to manage and execute a variety of tasks.
