Job Overview

We Fairview are looking for a Transplant Coordinator to join University of Minnesota Physicians Administration team in Minneapolis, MN! This role is casual/on-call.

The Transplant Coordinator is responsible for coordination and preparation of patients for organ transplantation and the long-term management of patients after transplant through collaboration with various members of the interdisciplinary team.

Responsibilities:

+ Coordinate patient care in both pre- and post-transplant phases

+ Collaborate with interdisciplinary team members within the organization and outside community partners

+ Act as a liaison for transplant physicians to implement medical management of conditions

+ Provide telephone triage and make patient care decisions based on patient symptom assessment and evaluation, laboratory results, and patient condition changes

+ Apply appropriate medical interventions and transplant protocols based on patient assessment, laboratory results, and patient condition changes

+ Assumes on-call responsibilities after office hours by providing a resource for medical triage. Acts as transplant coordinator when donor organ becomes available.

+ Provides on-going education to patients and families

+ Collaborate with other members of the clinical team to improve patient care and benchmarks through quality improvement projects

+ Participate in the development and revision of protocols

+ Maintain knowledge of current practices in transplantation, nursing, and healthcare

Required Qualifications

+ Bachelors Degree in Nursing orAssociates Degree in Nursing

+ 3 years acute care experience in ICU, CCU, Transplant, Acute Care Med-Surg or 5 years acute care experience in ICU, CCU, Transplant, Acute Care Med-Surg

+ Registered Nurse

+ Basic Life Support (American Heart Assoc, Red Cross or MTN)

Compensation Disclaimer

The posted pay range is for a 40-hour workweek (1.0 FTE).
