Job Title: Chemical Dependency Counselor

SUMMARY:
Performs all functions of counselor practice as outlined by NYS OASAS. Focus includes the development and implementation of individualized patient treatment plan with emphasis on counseling and guidance of recovery process.
RESPONSIBILITIES:
+ Works collaborative with medical team to develop treatment planning strategies.
+ Responsible for making appropriate referrals and discharge planning of patients.
+ Ensure accurate and timely documentation of patient records.
+ Participate in daily group counseling efforts of the unit.
+ Responsible for acting as the primary Individual Counselor for patients in their recovery process.
REQUIRED QUALIFICATIONS:
+ Two years of college with specialization in chemical dependency or other human services area
+ BLS Required

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant.
