SUMMARY
Provides evaluation, diagnosis, and treatment of patients with problems in cognition, communication, and/or deglutition using accepted techniques and equipment within the guidelines of the organization's policies and procedures and professional standards of practice. Assesses and restores patient's function, alleviates pain, and prevents disabilities by designing, planning and implementing medically prescribed speech-language pathology interventions. RESPONSIBILITIES
+ Meets the patient's goals and needs and provides quality, evidence based care by assessing, developing and implementing an individualized treatment plan.
+ Helps patient accomplish treatment plan and accept therapeutic devices and recommendations by instructing, encouraging, and assisting patients in performing functional and daily-living activities and in using assistive and supportive devices.
+ Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societies.
+ Develops speech-language therapy staff by providing information; developing and conducting in-service training programs.
+ Complies with federal, state, and local legal and professional requirements by studying existing and new legislation; anticipating future legislation; enforcing adherence to requirements; advising management on needed actions.
+ Contributes to team effort by accomplishing related results as needed.
+ Maintains a caseload of clients, as assigned, which supports the standards productivity requirements of the program. Coordinates client service request and / or referrals in an efficient and timely manner.
+ Provides comprehensive skilled therapy assessments, formulates a comprehensive plan of home care services for assigned clients based on skilled therapy assessment. Performs skilled therapy and prescribed treatments to assigned clients based on MD orders. Orders, obtains prescribed equipment as needed and teaches clients regarding the equipment obtained.
+ Communicates plan of care to the Primary Care Physician (PCP), updates PCP with changes in condition and subsequent alterations to plan of care. Documents and implements physicians orders per D.O.H., Federal regulations and program standards. Provides client and family education in accordance with client assessment and prescribed plan of care.
+ Establishes paraprofessional care plan as needed and supervises paraprofessional per DOH regulation. Completes accurately and timely all documentation of assigned clients’ care per regulatory standards.
REQUIRED QUALIFICTIONS:
+ Masters degree in Speech/Language Pathology (may be MA, MS, Med)
+ New York State licensure
+ Current N.Y.S. Drivers License is required to drive own car in order to meet essential functions.
PREFERRED QUALIFICTIONS
+ 1 year experience in medical setting preferred.

MS: Speech Pathology (Required)
LICENSES / CERTIFICATIONS:
CCC-SLP - Certificate of Clinical Competence in Speech-Language Pathology - American Speech-Language-Hearing Association (ASLHA), SLP - Speech-Language Pathologist - New York State Education Department (NYSED)
PHYSICAL REQUIREMENTS:
M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Pay is determined by factors including experience, relevant qualifications, specialty, internal equity, location, and contracts.
