Job Summary and Qualifications

The Sepsis & Stroke Coordinator is a health care professional who works in collaboration with the medical and hospital staff to provide multidisciplinary care to the septic and stroke patient. The Coordinator provides clinical and professional expertise for patients, families and staff members. Scope of practice includes providing organized care to the patient with sepsis with focus on health promotion and disease prevention. The Coordinator also functions as an educator and coordinates care in critical care/sepsis patient care management/stroke patient management, ensuring optimal patient care management and outcomes. The Coordinator will collect and analyze date surrounding the sepsis and stroke patients to assist with documentation, coding, clinical care, and outcomes. Functions in a professional self-directed manner to assure that position responsibilities are met. Applies good verbal and non-verbal skills to achieve service goals of customer service and satisfaction. Collaborates with all members of the inter-disciplinary team to achieve productive movement of the best practices within the organization. As requested, actively participates in accomplishing Facility, Division and Market Sepsis/Stroke service goals.

What qualifications you will need:

+ Current Florida license as Registered Nurse or Recognized Compact Licensure (subject to Florida State Licensing Requirements, including/not limited to ongoing eligibility and duration provisions)

+ Graduate from an accredited School of Nursing

+ Bachelor’s degree required, BSN preferred

+ Minimum of 3 years' experience in a progressive care unit

+ Meditech experience

We are an Agency for Health Care Administration (AHCA) Primary Stroke Center. HCA Florida Palms West Hospital has emergency services, a dedicated New Life Center and a Level II NICU. We have a full-service Children’s Hospital. Services include outpatient rehabilitation, an aquatics center, orthopedic spine center, robotics, and interventional radiology. HCA Florida Palms West Hospital is located on a 94-acre campus in Palm Beach County. The surrounding areas include Wellington, Royal Palm Beach and Loxahatchee.
