Care Coordinator - Haitian Creole

Lasante Health Center is seeking a Care Coordinator to join our team. The ideal candidate will monitor our patients’ medical treatment and facilitate communication with healthcare providers to guarantee access to high-quality medical care.

Responsibilities:

+ Regularly monitor and manage a designated group of patients with chronic medical conditions.

+ Enroll patients in different medical programs

+ Engage in consultations with patients and their family members to address health concerns.

+ Provide comprehensive education to patients regarding their medical conditions, medications, and specific care protocols.

+ Assist patients in scheduling appointments

+ Complete other tasks as directed by supervisors.

Qualifications:

+ Customer service experience

+ Computer skills

+ Ability to handle multiple tasks concurrently

+ Care coordination experience preferred
