    Job Summary and Qualifications

     

    The Director of Hospital Case Management (HCM) Operations will be a Subject Matter Expert (SME) and lead cross-functional teams within and outside from the Case Management department to develop and operationalize comprehensive programs to achieve desired strategic objectives. The Director will collaborate with highly clinical teams and leadership including: physicians, nursing, pharmacy, division VPs, and others. The Director will identify opportunities and facilitate process standardization, while optimizing technology efficiencies and reporting capabilities in a metric driven environment. The Director of Hospital Case Management Operations is responsible for the assessment, planning, coordination, development, and direction of Hospital Case Management initiatives as assigned. This position will serve as virtual onsite support to select divisions/markets as needed. This position requires up to 50% travel to HCA facilities. This position requires an individual who is a self-starter and thought-leader, has the ability to manage multiple priorities, work with minimal supervision on assigned projects and activities, demonstrates tact and diplomacy in situations of conflict and controversy and demonstrates excellent communication and presentation skills. This individual must be able to adapt quickly to change and be able to coordinate efforts across multiple stakeholders. The Director must understand the clinical and financial aspects of case management and must be comfortable working with all levels of management. The Director will work directly with the Divisional Leadership team and Group Vice Presidents

    Major Responsibilities:

    + Provides virtual onsite support to multiple markets/divisions

    + Complete Ad hoc projects as needed.

    + Provides support to Facility and Division leadership, and identifies areas of opportunity for continuous improvement including ongoing education and resources as needed

    + Applies InterQual criteria, determining current / proposed level of care.

    + Identifies and proposes strategies to address barriers to care progression and discharge planning, including financial, emotional, psychosocial, medical, and spiritual on reviewed cases.

    + Provides subject matter expertise and oversight in complex care management and discharge planning.

    + Works collaboratively with members of the case management team to address issues that arise through the complex patient escalation process.

    + Assists in toolkit development, training, and implementation as assigned.

    + Participates on length of stay projects by providing analysis of current state, opportunities, and mitigation recommendations based on research.

    + Effectively manages tasks and priorities in order to complete objectives timely.

    + Establishes and maintains effective and positive relationships which promote teamwork

    + Other duties as assigned

    What qualifications you will need:

    + Strong clinical background with 4 years acute care nursing experience, 2-3 years critical care experience preferred.

    + 4-5 years recent Case Management experience required with a strong emphasis on care coordination (progression of care), complex care and discharge planning.

    + Knowledge and understanding of governmental and Joint Commission regulations.

    + Ability to instruct in the approved methods, procedures and practices employed in effective care coordination and discharge planning.

    + Competent in the application of InterQual criteria for progression of care.

    + Strong knowledge of post-acute requirements and available community resources.

    + Ability to research national and community programs available to patients with limited or no resources.

    + Ability to analyze data and employ approved management techniques and statistical tools to obtain maximum effectiveness, efficiency and information.

    + Knowledge and understanding of standard hospital requirements / strategies / resources used to manage patients with extended length of stay and how to leverage these resource (UR Committee, CFO, Physician Advisor, CMO, Legal, Ethics, Palliative Care, etc.).

    + Ability to establish and maintain collaborative and effective working relationships.

    + Ability to communicate effectively in oral, written and electronic formats.

    + Strong analytical and critical thinking skills.

    + Must be able to gain proficiency in use documentation systems to Midas, Meditech, and other electronic medical records

    + Proficient in Microsoft Office Suite

    Education & Experience:

    + Bachelor's Degree in Nursing required

    + Master's degree preferred

    + 5+ years of experience in acute care nursing required

    + 3+ years of experience in Case Management required

    + The job may require travel up to 50%
