    Job Summary:

    Performs the functions of a Licensed Practical Nurse in assessing, planning, implementing, and evaluating assigned clients’ care in collaboration with the RN Nurse Coordinator. Requires a high degree of independent judgment.

    Qualifications:

    Graduate of an accredited school of nursing as a Licensed Practical Nurse (LPN)

     

    Unencumbered Nursing license.

     

    Uniform Assessment Instrument (UAI) and Individual Service Plan (ISP) trained or obtain within six months ofemployment

     

    Minimum three (3) years supervisory experience as a nurse

     

    CPR/First Aid Certified.
