The Service Coordinator is responsible for coordinating and maintaining support services for individuals enrolled in the YouPLUS (RW Part B Supportive Services Program) and/or HOPWA program services. The Service Coordinator will focus on increasing access to a centralized service model offering wrap-around services. The Service Coordinator reports to the Director of Supportive Services, who shall determine specific responsibilities of this position. These responsibilities may be revised, as necessary, with proper notification and training provided, where appropriate. The Service Coordinator will focus on patients with housing and case management priority needs.
As part of the essential functions of this role, the Service Coordinator:
+ Operates with a team approach to provide comprehensive support services addressing needs identified by patients
+ Participates in weekly team huddles and case conferences to address and work to resolve patient barriers
+ Contributes to the central intake and eligibility process by completing required documents to successfully enroll and retain eligible patients
+ Completes intake/assessment activities with patients upon enrollment and reassesses every 6 months to ensure patients are still eligible to receive services
+ Develops and implements patient-focused service plans to address and eliminate patient barriers to maintaining stable HIV care/treatment
+ Maintains timely data entry in EHR, Medent, to reflect all services provided. This includes encounters, referrals, receipts, transportation logs, etc.
+ Conducts housing search, initial and annual inspections and program-specific housing documents to support housing stability with the patient
+ Assists with securing services that maximize housing stability for patients: increase their ability to maintain their household and finances, independently perform activities of daily living, uphold the terms of their lease; prevent lease violations; intervene and mitigate crisis situations; maximize tenant safety and security; provide advocacy with landlords; and prevent avoidable evictions
+ Conducts regular home visits for patients receiving housing subsidies and/or as indicated in service plan
+ Provides quality customer service through a Trauma-Informed Care (TIC) lens
+ Contributes to monthly reporting by providing status reports for service utilization and achievement of program indicators
+ Contributes to in-reach and outreach efforts for recruitment of eligible patients
+ Contributes to quality care and services throughout the team by participating in quality meetings and quality improvement projects that enhance the patient experience and improve program success
Qualified candidate will have a BA/BS in Human Services or related field and one (1) year of qualifying related experience, OR Associate degree in Human Services or related field and two (2) years of qualifying related experience; experience developing and implementing direct client services and case management services preferred; candidate must have strong written, verbal, organizational, customer service, and time management skills;candidate must have a valid driver’s license and ability to travel between offices/home visits/other destinations as necessary; candidate must demonstrate an awareness of and sensitivity to the issues that underserved populations, people living with HIV and people who use drugs face, including health disparities that affect people of color at a disproportionate rate.
Job type: Full-time, 40 hours per week
Required education and experience: BA/BS in Human Services or related field and one (1) year of qualifying related experience, OR Associate degree in Human Services or related field and two (2) years of qualifying related experience. Experience developing and implementing direct client services and case management services preferred.
Additional requirements: Valid NYS driver’s license and insured, dependable car
