    The Structured Day Program Manager plays a key role in the success of the Acquired Brain Injury (ABI) Program by supporting the daily operations of the Structured Day Program (SDP). The SDP offers a combination of one-on-one and group-based services tailored to individuals with traumatic brain injuries (TBIs). These services focus on developing life skills, enhancing interpersonal relationships, and promoting self-expression through art and music. Programming is also designed to support cognitive rehabilitation and foster greater independence in daily living.

     

    The SDP Coordinator ensures effective service planning, day-to-day coordination of care, and regulatory compliance. The SDP Manager serves as a key liaison between participants, family, staff, service coordinators, and leadership to ensure high-quality service delivery, regulatory compliance, and participant-centered support.

     

    + Develop and maintain weekly program schedule in collaboration with staff, ensuring service delivery targets and staffing ratios are met

    + Ensure that SDP groups have coverage when staff are out

    + Assist with intakes and referrals to the Structured Day Program

    + Provide one-on-one support to ABI participants as needed

    + Facilitate designated SDP groups

    + Oversee quality of SDP groups and problem solve issues that arise

    + Provide staff with training on Netsmart and other systems

    + Provide staff with guidance and problem-solving support to address day-to-day problems related to service provision, schedule, and unforeseen circumstance

     

    Service Planning & Participant Support

     

    + Assist Service Coordinators with steps to update Service Plans, including completion of required forms and coordination of transportation

    + Coordinate staff attendance of 30-day and mid-service team meetings

    + Coordinating participant transportation through Medical Answering Services (MAS)

    + Develop individualized safety plans for participants and ensure plans are carried out by SDP staff

    + Conduct staff meetings/send emails necessary for effective/safe daily functioning of the program

     

    Documentation & Compliance

     

    + Coordinate completion of Individualized Service Reports (ISRs)

    + Review and approve daily case notes for accuracy and completeness

    + Document meeting and group notes within three days of service delivery

    + Monitor authorized units and ensure that service plans contain the necessary amount for each participant

     

    Administrative & Fiscal Oversight

     

    + Complete purchase requisitions for group activities as needed

    + Maintain accurate petty cash records and perform routine audits

    Requirements

    + Must meet the educational and experiential requirements for Structured Day Program Staff as outlined in the NYS Dept. of Health HCBS TBI Waiver Manual:

    + Be at least 18 years old with High School Diploma or Equivalent (i.e. GED)

    + Strong time management skills and flexibility in scheduling

    + Have the ability & skills to ensure the welfare & safety of participants

    + Ability to work independently with minimal supervision, as well as collaboratively within a team

    Preferred Qualifications/Skills:

    + Associates or Bachelor’s Degree in related field

    + At least two years working with individuals with disabilities and/or program coordination

    + Knowledge and understanding of the New York State Traumatic Brain Injury Waiver

    Physical Requirements:

    + Assist participants if needed when navigating SDP program environment if safety looks compromised on a given day.

    Salary Description

    $27/hour
