Job Description:
Utilizes advanced health assessment, knowledge, and decision making skills, works independently and in collaboration with physicians and other health care professionals. Provides health care to individuals and families, emphasizing health promotion and disease prevention. They may care for patients ranging in age from newborn to elderly as noted in age-related category and area of expertise.

Essentia Health is seeking a compassionate Palliative Care APP to join our expanding Palliative Care Program. This is a unique opportunity to be part of a collaborative, interdisciplinary team dedicated to delivering holistic, patient-centered care that supports individuals and their families through some of life’s most meaningful moments.
PRACTICE SPECIFICS
+ Full-time; Mon-Fri
+ Play a critical role in delivering comprehensive care to patients with serious illnesses.
+ Deliver care within the inpatient setting.
+ Close collaboration and support within the interdisciplinary palliative care team across the inpatient and outpatient settings which includes 1 Fellowship Trained Physician, 1 Social Worker, and 1 chaplain.
+ Opportunity to participate in the ongoing growth and development of the palliative care program.
+ EPIC (Electronic Medical Record) for both our Inpatient and Ambulatory facilities

REQUIREMENTS
•** **Significant experience in palliative medicine, oncology, hospice or geriatrics is strongly preferred.

+ Centrally located on the border of North Dakota & Minnesota
+ 3.5 hours northwest of Minneapolis/St. Paul
+ Fargo-Moorhead population: 225,000; Regional Population: 478,000

Licensure/Certification Qualifications:
NP:
Current licensure as a registered nurse in appropriate state.
Current licensure as a Nurse Practitioner in appropriate state.
Current certification in CPR.
National certification in applicable area.
Successful completion of Essentia Health credentialing process prior to practice
PA:
Current licensure as a physician assistant in appropriate state.
Current certification in CPR.
National certification.
Successful completion of Essentia Health credentialing process prior to practice
