Job Title:HCBS Behavioral Health Provider Coordinator
Employment Type:Full-Time, Monday – Friday (8:30 AM – 4:30 PM)
Job Summary:
The HCBS Behavioral Health Provider Coordinator is a vital part of FLACRA’s Home and Community-Based Services (HCBS) team, delivering and coordinating behavioral health services for adults with Serious Mental Illness (SMI) and/or Substance Use Disorders (SUD) who are eligible through HARP and Managed Care Organizations (MCOs). This dual-role position combines direct service delivery with referral coordination and program tracking. Under the supervision of the Program Director, the Coordinator ensures timely communication with referral sources, tracks program data, and supports access to individualized services that foster recovery, independence, and community integration.
Must-Have Qualifications & Skills:
+ High School Diploma or equivalent
+ 1–3 years of relevant experience working with individuals with Substance Use Disorders (SUD) and/or Serious Mental Illness (SMI)
+ NYS Driver’s License and reliable transportation (community travel required)
+ Strong written and verbal communication skills
+ Organizational, time management, and problem-solving skills
+ Ability to work independently and collaboratively with care teams
Nice-to-Have Qualifications:
+ Certification as a Peer Specialist or Certified Recovery Peer Advocate (CRPA)
+ Bachelor’s degree in Human Services or related field
+ Experience with HARP care management, HCBS documentation, or MCO coordination
+ Familiarity with BH HCBS service lines and NYS Provider Manual
+ Data tracking or program coordination experience
+ We are also open to candidates with a combination of experience and education that meets the responsibilities of the role

+ Ability to engage and support culturally diverse individuals
+ Knowledge of recovery principles and local behavioral health resources
+ Capacity to remain calm in crisis situations and apply engagement strategies
+ Strong interpersonal and advocacy skills
+ Comfortable with documentation, data tracking, and service management
Job Requirements:
Key Responsibilities:
Program Coordination & Referrals
+ Coordinate all incoming HCBS referrals, assign cases for intake, and ensure timely communication with referral sources
+ Track service data, manage thresholds, and maintain accurate program reports
+ Collaborate with care managers and other team members to develop and update individual service and crisis plans
Direct Service Delivery
+ Deliver approved HCBS services including:
+ Psychosocial Rehabilitation (PSR)
+ Habilitation (HAB)
+ Family Support and Training (FST)
+ Educational Support Services (ESS)
+ Empowerment Services – Peer Supports (EMP)
+ Pre-Vocational Services (PVOC)
+ Serve as a role model and coach, promoting recovery and independence
+ Provide outreach, advocacy, transportation, and community-based support
Documentation & Compliance
+ Maintain accurate and timely documentation for all client interactions
+ Ensure services meet HARP/MCO guidelines and HCBS documentation standards
+ Participate in trainings required for HCBS program compliance and professional development
