    Responsibilities

    + Conduct appropriate evaluations, assess home environment and identify equipment needs relative to speech/language function

    + Identify functional speech defects and establish a plan of care to improve patient’s function

    + Administer speech therapy program utilizing specialized therapeutic technique and/or equipment and recommend communication devices/aids as indicated

    + Maintain accurate and timely documentation

    + Participate in case conference as required

    + Ensure compliance with all state and federal legal and regulatory requirements

    + Additional responsibilities as assigned

    Qualifications

    + Master’s degree in Speech Language Pathology with current licensure to practice in state of operation

    + 1+ year experience in Speech Language Pathology

    + Home health experience preferred

    + Commitment to clinical and documentation excellence
