Private Duty LPN – G-Tube Care (Hornbeak, TN)

Overview:Join our caring team as a Private Duty LPN, providing in-home care split betweenHornbeak, TN, and Jackson, TN.You’ll work thefirst half of the month in Jackson and the second half in Hornbeak, enjoying variety while making a meaningful difference.

Location & Schedule:

+ Hornbeak, TN: 15th through the 30th of each month

+ Week 1: Monday, Tuesday, Thursday, Friday (7:30am–5:30pm)

+ Week 2: Monday, Tuesday, Saturday, Sunday (7:30am–5:30pm)

+ Jackson, TN: 1st through the 15th of each month

+ Same schedule split applies

Qualifications:

+ Current LPN license

+ G-Tube care experience required

+ CPR certification

Responsibilities

+ Perform selected nursing procedures within the scope of LPN practice

+ Provide patient and family teaching according to plan outlined by the RN

+ Monitor patient's vital signs and make specific measurements and observations

+ Monitor and record intake and output. Report significant changes immediately

+ Observe, assess and record patient's physical and mental condition and reports any changes

+ Administer medications as instructed and as ordered by the physician and instructed by the RN and document the patient's file accordingly

+ Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned

Qualifications

+ Licensed Practical Nurse / LPN license in good standing

+ Previous private duty, home health, hospice, hospital or skilled nursing experience preferred

+ Active CPR certification

+ Familiarity with electronic health records

+ Strong problem solving and critical thinking skills

+ Reliable transportation, valid driver's license and current automobile insurance
