    + $35-$45 per Hour

    + Physical Therapist Role

     

    CONTRACTOR shall provide qualified NYS licensed physical therapist(s) to perform physical therapy consultant services on site for Bronx Psychiatric Center (BPC) clients/ patients.

     

    The services will be provided for an estimated 20 hours per week.

    The NYS licensed physical therapist shall:

    A. Act as physical therapy consultant.

    B. Assess and evaluate client’s condition in order to develop a physical therapy treatment plan based upon a physician's request.

    C. Review clients treatment goals, treatment and medical history.

    D. Perform and interpret standard muscle, nerve, joint and functional ability tests.

    E. Administer appropriate physical therapy treatment when indicated.

    F. Prepare documentation of assessment, evaluation, delivery of treatment services and patient progress for client's medical record.

    G. Consult with client's treatment team as needed.

    H. Be under the supervision of Bronx Psychiatric Center Director of Medicine or the Director's designee.

    I. Conduct a brace/shoe clinic as needed. nerve, joint and functional ability tests.

    J. Administer appropriate physical therapy treatment when indicated.

    K. Prepare documentation of assessment, evaluation, delivery of treatment services and patient progress for client's medical record.

    L. Consult with client's treatment team as needed.

    M. Be under the supervision of Bronx Psychiatric Center Director of Medicine or the Director's designee.

    N. Conduct a brace/shoe clinic as needed.

    O. Evaluate patients for wheelchairs and adaptive equipment.

    P. Submit to the Medical Director and Business Office: monthly statistical information including number of patients

     

    treated, number of physical therapy modalities administered, number of evaluations and number of admissions/discharges in the physical therapy services.

     

    Q. Maintain a performance improvement program related to structure, process and outcomes.
