Job Summary
Good Samaritan Society is seeking a hospice chaplain/bereavement coordinator to join our team. This role involves developing, implementing and coordinating the Bereavement Care Program and focuses on the family and patient experience. Responsible for developing an individualized plan of care for families to cope with physical, emotional, and spiritual needs. Provides spiritual care and counseling to patients and families based on individual preferences and needs, offering comfort and guidance during and after end of life care. The Chaplain/Bereavement Counselor completes spiritual assessment and creates personalized care plans to fit the needs of the family and patients. Serve as a liaison and collaborate with the multidisciplinary team to provide holistic care including emotional, physical and spiritual support.
Coordinates the logistical implementation of the program, and adheres to program actions and deliverables based on program grants when relevant. Manages multiple concurrent projects and/or priorities. Establishes project plans that identify, communicate, and track tasks and deliverables to meet goals/objectives. Evaluates key metrics related to project goals/objectives. Coordinates communication and related committee meetings with internal and external partners. Utilizes interdependence in the workplace and on work projects within the department. Assesses and monitors potential or actual problems and implements resolutions needed for successful outcome. Respects and maintains confidentiality. Provide spiritual care and counseling to patients and families based on individual preferences and needs, offering comfort and guidance during end-of-life care.
Work hours vary according to department and client needs. May be required to work evenings and/or weekends.
Collaborates with Physician Hospice Care provider, Hospice Case Manager, and Social worker for initial consult and Spiritual Care to provide grief and bereavement support services to patients and families affected by trauma or serious illness and death.
Qualifications
Bachelors degree required in related work field (i.e. Child Development, Family Science, healthcare); Masters degree preferred.
Clinical licensure may be required dependent on patient population and department specialties, i.e. Certified Child Life Specialist in pediatrics, End-of-Life Nursing Education Consortium (ELNEC) credential in palliative care.
