Description: The ABH Coordinator for Collaborative Care and BH Integration will provide direct clinical and operational supervision to the Collaborative Care Program, based in several Atlantic Medical Group Practices throughout the AHS regions of service. The position will also provide assistance in the clinical and operational supervision and management of the Hospital Elder Life Program and Peer Recovery Program at Morristown Medical Center. The position will work collaboratively with the ABH Manager for Clinical Integration, Director for Clinical Integration and Hospital Based BH Services and the System Medical Director of Psychiatry.

Job Function: 01. Strategic Planning: Develops, refines and manages initiatives to enhance access to the Collaborative Care (CoCM) program in coordination with the Service Line Manger, Director and System Medical Director for Psychiatry. Collaborates with practice managers, Senior Managers and AMG leadership to evaluate clinical and fiscal viability of current and/or new operations. Age Specific Competencies: Child, Adult. Geriatric
Job Function: 02. Strategic Planning: Assists, monitors and collaborates with programmatic Epic optimization projects in collaboration with Service Line Manager and Director. Designs, implements and monitors workflow and programmatic implementation projects in coordination with Service Line Manager and Director. Completes training for CoCM team pertaining to workflow and program process changes. Validates and monitors data integrity for program KPIs and metrics. Age Specific Competencies: Child, Adult. Geriatric
Job Function: 03. Strategic Planning: Identifies program growth needs in both current and potential future medical practices in coordination with Service Line Manager and Director. Provides onsite in-person program interface by traveling to CoCM based medical practices to assist with program pre-implementation, implementation and monitoring. Assists directly with potential performance, clinical and hiring needs for both direct reporting and direct supported CoCM practices. Interfaces with practice managers, senior managers, AMG leadership and practice medical leads. Maintains consistent contact with practice administrators both in-person monthly and virtually monthly.
Job Function: 04. Clinical: Establishes, implements and monitors program clinical standards, KPIs, and treatment strategy in coordination with Service Line Manager and Director. Conducts individual and group supervision for Collaborative Care Clinicians. Develops, organizes and executes clinical training for CoCM clinicians in coordination with the Service Line Manager. Establishes and maintains clinical communication and clinical review with CoCM collaborating psychiatrists. Age Specific Competencies: Child, Adult. Geriatric
Job Function: 05: Clinical: Provides direct clinical care, as needed, both assessment and treatment. Assesses and confers with supervising psychiatrist to ensure appropriate diagnoses. Establishes treatment goals and plan with the patient and family, as appropriate, and in coordination with collaborating psychiatrist and primary/specialty providers. Applies evidenced-based program supported clinical interventions. Coordinates discharge planning and care coordination needs post discharge for patients. Works collaboratively with referring medical providers, collaborating psychiatrists and Service Line Manager and Director. Age Specific Competencies: Child, Adult. Geriatric
Job Function: 06 Performance Improvement: Develop, design and monitors program related KPI and metric tracking and reporting in coordination with Service Line Manager and Director. Completes data analysis and reporting for program KPIs, metrics and analytics. Collaborates with AMG, BH and program leadership to identify clinical, operational, financial and program outcome measures and research. Collects, designs, authors and reports on PI/QI projects, program outcomes and KPIs in coordination with Service Line Manager and Director. Completes Lean/project management training and PI projects.
Job Function: 07. Reporting: Attends and presents in program-based and service line meetings and reporting events. Authors and edits reporting for QI/ PT metrics in coordination with Service Line Manager, Director and System Medical Director. Attends presentation events pertaining to the program and actively presents/participates in education and program related activities.
Job Function: 08. Fiscal: Reviews and provides feedback related to program based financial KPIs with Service Line Manager, Director and AMG Leadership. Implements and monitors programmatic adjustments determined through review of financial metrics in coordination with Service Line Manager and Director. Monitors operational and clinical outpatient to assist in optimizing the program and practice workflow in response to financial KPIs in coordination with Service Line Manager and Director. Monitors regulatory requirements with various governing entities for billing and clinical operation compliance in coordination with Service Line Manager and Director.
Job Function: 09. Documentation: Completes all required clinical documentation in the required completion timeframe established by the Service Line Manager and Director. Completes, documents, and provides feedback to CoCM clinicians from monthly conducted chart reviews for various practices. Monitors CoCM clinician documentation integrity, time to completion rate and accuracy in conjunction with regulatory and programmatic guidelines. Monitors and provides feedback for documentation impact on data and reporting integrity. Coordinates and implements workflow documentation changes with Service Line Manager and Director. Ensures all documentation reflects clinical interventions in accordance with diagnosis, treatment goals and criteria for discharge. Age Specific Competencies: Child, Adult, Geriatric
Job Function: 10. Team Management: Supports and develops team member professional growth in coordination with Service Line Manager. Completes critical conversations with team members, provided direct growth and development guidance and additional performance improvement interventions as required and in coordination with program, departmental, service line and system guidelines, protocols and policies. Assists with interviewing, hiring, orienting, training, and supervising all staff assigned. Initiates disciplinary action/performance improvement plans as needed. Monitors, designs, and revises staff competencies. Completes all performance evaluations. Collaborates with Service Liner Manager and Director about team member needs and development.
Job Function: 11. Strategic planning: Assists in the development and operation of the Hospital Elder Life Program and Peer Recovery Program at Morristown Medical Center, by working directly with the clinician and peer teams in the development of patient services and service delivery workflows.
Job Function: 12. Clinical: Assisting in the completion of case reviews, direct supervision and patient care outcomes with direct care clinical and peer team members.
Job Function: 13. Reporting: Assisting in the development, design, maintenance and reporting of quality, performance and KPIs for Hospital Elder Life and Peer Recovery at Morristown Medical Center. Creating EHR based reporting, data validation and report presentations.
Other tasks assigned as need.
Required:
1. Master's or Doctoral degree in Mental Health discipline or related field.
2. 5+ years of experience in Mental Health/Behavioral Medicine or relevant area.
3. 2 years of experience in clinical supervisory and service management.
3. NJ state licensure in LPC, LCSW or Psychologist.
Preferred:
1. Experience working in medical settings: ED, inpatient medical units, primary/specialty care practices.
