ORGANIZATIONAL DUTIES AND RESPONSIBILITIES
1. Patient Family Centered Care
Following are examples of criteria for areas that normally do not have direct patient/family contact:
Ensures that all patient inquiries are answered with courtesy and promptness; offers assistance to clinical staff, as appropriate and within scope of practice.
Consistently found assisting patients/families and other visitors, always ensuring all interactions with NLH create a positive reflection on our facility. Promotes a warm and welcoming environment, in and outside of home department and role. Offers ideas for improvement and participates in task forces, committees, and related projects focused on improving the patient/family experience. Proactively addressing potential concerns/questions.
• Incorporates patient, family, and caregiver as defined by the patient and other team members when planning care and service delivery;
• Demonstrates an attitude that creates an atmosphere of respect for human differences, beliefs, values and the uniqueness of each person encountered;
• Shares complete and unbiased information with patients and families, with patients consent.
• Partners with patients and families to allow for effective decisions and participation in care.
2. Health and Safety
• Demonstrates safe behaviors by following and coaching others in regard to safety practices, procedures, and using required Personal Protective Equipment (gloves, gowns, masks, etc.).
• Fosters a safe work environment by eliminating or reporting hazards.
• Reports incidents in a timely manner to ensure quick medical treatment and processing.
• Follows infection prevention control policies and procedures, for the protection of oneself and others.
• Adheres to hand hygiene procedures and expectations.
3. Professional Conduct
• Adheres to Hospital Policies, Procedures, and Code of Conduct, including all IS specific policies and procedures, which prohibit the use of our Information System for any use other than the sole purpose of completing work related tasks.
• Maintains strict confidentiality of all patient and business information.
• Accesses only the information needed with the sole purpose of completing work related activities.
• Is respectful and trustworthy of others throughout the organization.
• Is committed to organization-wide professional and personal development in reaching NLHA's goals.
• Communicates effectively with others.
• Makes decisions based on best practices and evidence-based information.
4. Professional Responsibility
• Completes competency skill checklists and mandatory education/certification programs and other mandatory documentation. Submits documentation to HR for permanent file.
• Enhances professional growth through participation in educational programs and workshops.
• Maintains licensure and certification.
• Uses resources provided by the organization for communication, documentation, and to complete other requirements of the position.
• Able to articulate department and institutional quality goals and initiatives.
• Demonstrates fiscal responsibility and good stewardship of resources.
5. Quality
• Actively involved in quality and process improvement initiatives in the department and/or organization.
FUNCTIONAL DUTIES AND RESPONSIBILITIES
1. Clinical Practice:
• Conducts physical exams, takes medical histories, diagnoses and treats common acute minor yet urgent illness or injuries
• Plans and implements appropriate treatment plans, evaluating responses to interventions, and revising plan based on overall effectiveness
• Orders appropriate ancillary tests, and interprets them in a timely manner including EKG interpretation
• Instructs clinical support personnel assigned to assist in patient care
• Provides education to patients and families based on assessment of learning needs
• Comfortable performing procedures such as laceration repair, foreign body removal, I+D of abscesses, splinting common fractures, and epistaxis management
• Ensures an appropriate disposition and follow up plan for patients
• Must be able to manage multiple patients at once
• Must work well autonomously
• Comfortable treating patients ages 2 two (2) years and older
2. Administrative Practices:
• Adheres to the established administrative practice standards and procedures of New London Hospital and express care practice guidelines
• Documents care provided using practice standards utilizing the electronic health record
POSITION REQUIREMENTS
• Responds to all progress notes from office staff in a timely manner, including all out-going correspondence and referrals
Ideal candidate will be a Family Nurse Practitioner or Physician Assistant with:
• Nurse Practitioner: RN and APRN currently licensed or eligible for licensure in NH
• Physician Assistant: Graduate from a certified Physician Assistant Program. Current certification or eligibility for certification to practice as a Physician Assistant in the state of NH
• Active DEA License
• Previous experience in Urgent Care or Emergency Medicine. 1-2 years preferred
• Board Certified with respective organization
• Able to work in a high pace/high volume setting independently
• ACLS
• ATLS preferred
• NH license in good standing
