Responsibilities
POSITION SUMMARY: The Ongoing Service Coordinator’s principle responsibility is to ensure that children and families receive the services to which they are recommended for and granted under the Early Intervention Program. The Ongoing Service Coordinator assists the family throughout the initial referral process, in receiving all services identified in the IFSP; coordinating these services and ensuring that they are delivered in a consistent and timely manner, and referring to additional services which may help the child or family as needed. This position serves children ages birth to 3 years who receive services through the Early Intervention Program (Nassau and Suffolk Counties).
RESPONSIBILITIES/ACCOUNTABILITIES:
Coordinates and monitors the delivery of Early Intervention services in accordance with the EIP timelines, policies, and procedures for initial referral pre-IFSP, & ongoing services, post IFSP through transition out of services or age out.
Provides assistance and services for an eligible child and the child’s parent or caregiver to receive the rights, procedural safeguards, and services authorized under the Early Intervention Program.
Facilitates opportunities for the provision of collaborative, family-centered services for infants and toddlers with disabilities and their parents or caregivers.
Guides families to identify and prioritize concerns, assists parents and caregivers to develop goals for functional outcomes, supports families to implement strategies, and aids them to identify resources to help meet the needs of their children and family units, which empowers families by strengthening their competencies and knowledge.
Acts as a liaison, coordinating services across agency departments, serving as the primary point of contact in helping parents or caregivers obtain the information, services, and assistance they need.
Reviews Early Intervention Evaluations.
Assists in preparation for audits.
Collects and audits case documentation and related files.
Maintains patient records in the EMR system and NYEIS system.
Ensures ongoing communication with parents and caregivers, referral sources, and the contracted municipalities through various modes of communication such as email, phone call, text message, and in-person meetings.
Conducts checks to ensure insurance eligibility and updates.
Coordinates documentation and communication to facilitate a smooth transition out of services at age-out period.
Travels as needed to attend meetings for initial contact visits, ongoing home visits, and IFSP meetings.
Performs other duties as assigned.
Qualifications
A minimum of one of the following educational or service coordination experience credentials:
Two years experience in service coordination activities (voluntary or part-time experience which can be verified will be accepted on a pro rata basis); or,
One year of service coordination experience and an additional year of experience in a service setting with infants and toddlers with developmental delays or disabilities; or,
One year of service coordination experience and an Associate’s degree in a health or human service field; or
A Bachelor’s degree in a health or human service field.
Service Coordinators shall participate in the introductory service coordination training session sponsored or approved by the Department of Health, in the first three months and by no later than one year of direct or contractual employment as an early intervention service coordinator, provided that training sessions are offered and accessible in locations with reasonable proximity to their place of employment at least three times annually.
Posted Salary Range
USD $22.50 - USD $25.00 /Hr.
