    Summary

    Provides professional patient care through the evaluation, planning, implementation, and documentation of physical therapy treatment. Delivers care in accordance with physician orders using a variety of therapeutic techniques and assistive devices to help patients reach their maximum functional potential. Develops individualized treatment plans with measurable, achievable goals and updates them as needed throughout the therapy process. Instructs and supervises Physical Therapist Assistants (PTAs) in implementing treatment procedures and ensures proper oversight of care. Educates patients, families, and caregivers on exercises and home programs to support continued progress after discharge. Completes all documentation and billing promptly and accurately. Care is delivered in alignment with hospital and department standards, the Indiana State Practice Act, the American Physical Therapy Association (APTA), and The Joint Commission.

    Education

    + Must be a graduate of an accredited physical therapy program.

    + Bachelor’s degree required if graduated before January 1, 2016.

    + Doctorate in Physical Therapy required if graduated on or after January 1, 2016.

    + International candidates must hold a minimum of a Bachelor’s degree in Physical Therapy.

    + Doctorate of Physical Therapy preferred.

    Licensure/Certification

    + Must hold or be eligible for a Physical Therapist license in the state of service, to be obtained within 90 days of employment.

    + CPR certification required within the first 90 days.

    + Must maintain all certifications and licensure per expiration dates.

    + APTA membership preferred.

    + Valid driver’s license required for those assigned to drive a company vehicle; proof of insurance required for personal vehicle use.

    Experience

    + Prior work experience preferred, but not required beyond clinical training within an educational program.

    Other Qualifications

    + Must be able to lift 50 pounds, assist with 100 pounds, and may need to push up to 300 pounds.

    + Must be flexible and willing to work at other locations as assigned.
