    **The Vascular Access Coordinator** is responsible for the coordination of care and management of patients’ vascular accesses and the communication and collaboration with the Vascular Access Team.

    **Schedule:** Full-time, Sundays off; no overnight shifts

    Responsibilities

    The following is a summary of what makes our team successful. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

     

    + Focus on vascular access care and management to increase permanent access longevity (AVF or AVG)

    + Maintain vascular access data to assure tracking and trending of dialysis therapy and outcomes monitoring for CQI / QAPI activities

    + Provide leadership and communication among caregivers

    + Educate staff and hemodialysis patients in the care and management of vascular accesses, with particular attention to opportunities for improvement.

    + Monitor vascular access and hemodialysis care delivery with particular focus on identifying access dysfunction and opportunities to increase arteriovenous fistulas (AVFs) and reduce catheters.

    + For accesses with dysfunction or other pathology, assist with referral for diagnosis or surgical treatment

    + Participate in Network 7 CQI activities to improve patient and facility vascular access outcomes.

    + Complete other vascular access-related tasks as directed by Nurse Manager.

    Qualifications

    + Six months experience as hemodialysis professional

    + Certification as hemodialysis technician or licensure as LPN or RN

    + Demonstrated proficiency in vascular access care and management

    + Demonstrated commitment to continuous quality improvement (CQI)

    + Excellent communication, writing, and organization skills

    + Basic computer skills
