    **Job Title:** Home Health Wound Care Registered Nurse (WOCN)

    **Location:** Philadelphia, Pennsylvania; occasional travel to surrounding counties may be required

    Requirements

    + **Education:** Graduate of an accredited school of nursing; BSN preferred

    + **Licensure:** Current and valid Registered Nurse (RN) license in the state of Pennsylvania

    + **Certification:** Certified Wound Ostomy Continence Nurse (CWOCN) certification required

    + **Experience:** Minimum of two years of experience as a Registered Nurse in a relevant clinical setting; prior experience in wound care, ostomy care, and/or continence management preferred

    + **Skills:** Excellent assessment, critical thinking, and problem-solving skills; strong interpersonal and communication skills; proficiency in electronic medical records systems; ability to work independently and as part of a team; current CPR/BLS certification; valid driver's license and reliable transportation

    Position Details

    + **Responsibilities:**

    + Conduct comprehensive assessments of patients' wounds, ostomies, and continence issues

    + Develop individualized plans of care in collaboration with physicians and interdisciplinary teams

    + Select appropriate wound dressings, therapies, and equipment based on evidence-based practice guidelines

    + Educate patients and families on wound care techniques, ostomy care, and continence management strategies

    + Provide direct patient care, including wound dressing changes, ostomy care, catheter care, and skin care

    + Monitor patient progress and adjust care plans as needed

    + Provide emotional and psychosocial support to patients and families

    + Participate in quality improvement activities related to wound care, ostomy care, and continence management

    + Maintain accurate and timely documentation of all patient assessments, interventions, and outcomes
