• Quality and Accreditation Post-Acute Specialist

    Holzer Health System (Gallipolis, OH)


    Under the direction of the Manager of Accreditation, the Post-Acute Specialist will coordinate, facilitate, measure, and evaluate both process and performance improvement initiatives and regulatory requirements for survey readiness at assigned facilities which may include Holzer Senior Care, Holzer Assisted Living, Holzer Home Health, and Holzer Hospice. They will establish and maintain effective relationships with a variety of agencies, organizations and individuals and prepare comprehensive and clear oral and written reports.

    Education, Work Experience and Licensure:

    • Associate degree from an accredited nursing institution required; Bachelor’s degree preferred.

    • Current Ohio Nursing licensure.

    • Minimum of 3 years’ experience in a long-term care, home care or an acute care facility.

    • Minimum of 1 years’ experience in leadership, preferably in healthcare.

    • Demonstrates a clear, working knowledge of home care operations and long-term care operations.

    • Demonstrates a clear, working knowledge of CMS, The Joint Commission, and Ohio Department of Health standards and requirements.
