Additional Information About the Role
Join the BJC Hospice team in providing bereavement support for families. You will support our north team in providing virtual, phone and home visits to follow up with families for 13 months.
The position will also lead grief support groups and participate in Stepping Stones camp as well as various support groups throughout the year.
Monday - Friday position with some evenings and weekends.
Overview
**BJC Home Care** offers patients and their families a complete range of home care services, including skilled nursing services, adult and pediatric hospice and supportive care, rehabilitation therapy, home infusion therapy, infusion treatment rooms, home medical equipment and high-tech respiratory care. Specialty home care programs also are available, including adult and pediatric asthma, cardiac, diabetes, orthopedic and wound care programs.

The BJC Home Care Services Supportive Care staff offer patients with life-limiting illness an opportunity to ease the transition from curative treatment to Hospice care through a unique program. Supportive Care offers a bridge between curative and terminal care. It is a multidisciplinary approach, holistic in nature, centered on the needs of the patient and patient's family, that provides coordinated medical, emotional and spiritual care.

Role Purpose
Responsible for working with adults and children to help them understand and cope with feelings of grief before and/or after the death of a loved one.
Responsibilities
+ Provides support to client's coping with their grief, and provides anticipatory and follow-up bereavement visits.
+ Evaluates grief resources and selects those to be utilized by BJC Hospice.
+ Coordinates grief activities.
+ Refers families to available services, counselors and support groups in the community.
+ Serves as a grief resource in the community and is involved in community organizations focused on education and support of pediatric and adult grief.
