LMSW Care Transition Manager/Social Worker/ PRN
**Work location:** Texas Health Denton 3000 N I-35 TX 76201
**Work hours:** PRN (As Needed) Shifts vary: Monday through Friday, Sat, Sun 8 am to 5 pm and Monday through Friday 10 am to 10:30 pm

This new building will bring the latest in obstetric care to North Texas as well as a new breast imaging center, outpatient physical therapy, cardiac rehabilitation and a 10-bed expansion to our emergency department.
Here’s What You Need
Master's Degree Social Work id required upon hire. (Individuals hired as a CTSW prior to May 11, 2017, will be grandfathered to the CTSW position with BSW, at the entity they were employed at on May 11, 2017)
Three (3) years of hospital/medical social work experience is preferred.
One (1) year of discharge planning/care management experience is preferred.
Licenses and Certifications
LMSW - Licensed Master Social Worker or LCSW - Licensed Clinical Social Worker is required upon hire.
CPR - Cardiopulmonary Resuscitation is required upon hire.
ACM - Accredited Case Manager is preferred upon hire.
CCM - Certified Case Manager is preferred upon hire.
Other ANCC is preferred.
What You Will Do
Delivers care to patients utilizing the Nursing Process
Responsible for ensuring patients are transitioned to appropriate levels of care in a timely and effective manner.
