Job Summary:
Key Responsibilities:
+ Coordinate patients interfacing with the CHAVAT, including reviewing charts for eligibility for the program, providing outreach to families, and coordinating intakes.
+ Conduct assessments and provide brief motivational interviewing interventions for youth at risk for are having already perpetrating violence.
+ Conduct MI-focused interventions focusing on ways to reduce both violence perpetration, gun carrying, and gun violence in the outpatient setting.
+ Communicate treatment plans to the other behavioral health specialists involved in the CHAVAT as well as coordinate appointments with youth and their families.
+ Coordinate wrap-around services, setting up appointments between youth identified in the emergency department or hospital setting for outpatient management.
+ Participate in the care of approximately 100 clients annually.
+ Other activities as deemed necessary by the supervisor.

Qualifications:
Minimum Qualifications:
+ Bachelor's degree in social work or a directly related field.
+ At least two (2) years of clinical experience working with adolescents and families.
+ Licensed Social Worker (LSW).
Conditions of Employment:
+ Must successfully pass a drug test through Children's Hospital Colorado.
+ _Must be willing and able to pass a national criminal background check._

_Preferred Qualifications:_
+ _Master's degree in Social Work (MSW)._
+ _Registered Nurse (RN) license._
+ _Licensed Clinical Social Worker (LCSW)._
+ _Ideally, but not limited to, at least 3 years of experience working with at-risk adolescents and young adults (substance use, juvenile justice, violence prevention)._
+ _Proficient in Motivational Interviewing skills._
+ _Experience with trauma-informed care._
_Knowledge, Skills, and Abilities:_
+ _Counseling skills._
+ _Efficient with organizational skills and basic computer skills._
+ _Excellent communication skills._
+ _Ability to work with a team of healthcare providers._
