Job Description
The Nurse Clinical Coordinator is a Registered Professional Nurse as well as a member of the palliative care interdisciplinary team who applies a multidimensional approach to the challenges that patients and families face coping with life-limiting/serious illnesses. The qualified candidate must possess broad clinical knowledge, demonstrate experience in palliative care, strong theory-based practice, and excellent interpersonal and communication skills when playing a pivotal role in the provision of palliative care consultations.
The goal of a Nurse Clinical Coordinator is to prevent and relieve suffering, supporting the best possible quality of life for patients and their caregivers-- regardless of the stage of the disease or need for other treatments--by coordinating clinical programs and services for the palliative care patient population.
Qualifications
Education Requirements
Bachelors degree in specific discipline (i.e., Bachelor of Science in Nursing) is required. Masters degree in health related field is preferred.
Certification in specialty field is preferred
Experience Requirements
2 years experience in specialty clinical field; 3 years preferred in palliative nursing,
Certified Hospice and Palliative Nurse (CHPN) required
Licensing and Certification Requirements (if applicable)
Current NYS nurse licensure
BCLS certification Required Issuing Authority: AHA

Responsibilities
+ Obtain a comprehensive view of the patient’s issues and circumstances, explore options for care, and advocate for the patient’s values, goals, and preferences by meeting with all patients and families cared for by the outpatient palliative care/supportive oncology teams.
+ Provide education to patients and families about medication (opioids and non-opioids) and symptom management to patients with advanced disease
+ In collaboration with the palliative care attending/nurse practitioners/fellows – communicate with patients and families outside of their scheduled appointments to assure quality of care and ongoing management of symptoms and medications. This includes returning and triaging phone calls to patients/families/clinicians.
+ Assist palliative care clinicians with gathering information required for prescription refills (e.g. gathering information from patients, pharmacies, providers).
+ Assist physicians with prior authorizations for medications and other approvals as needed for palliative related interventions or procedures.
+ Facilitate clarification of patients and family goals of care through collaboration with the patient’s healthcare team, ensuring all involved parties have congruent goals.
+ Initiate and/or collaborate with primary palliative care clinicians and other clinicians regarding Goals of Care/EOL discussions (e.g. HCP, DNR/DNR, Molst forms, isolated patient issues).
+ Act as consultative resource by demonstrating skilled and meticulous symptom management, with extensive involvement and participation in organizing outpatient, community care, discharge planning, and/or continuity of care efforts.
+ Enhance traditional disease-model treatments by promoting emotional, spiritual, practical, and complementary (e.g. music therapy, art therapy, etc.) supports to patients and families.
+ Serve as a role-model and mentor to physicians and nurses at all levels of training while providing active support, collaboration, and clinical expertise in rapidly changing circumstances.
+ Consistently demonstrate the role of patient advocate, protector of patient’s rights, and provider of psychological, social and cultural support.
+ Educate patients, families, and staff regarding the importance of palliative/end-of life options.
+ Manage clinic flow to maintain efficiency and a positive experience for patients and clinic team members, as well as reviewing schedules for accuracy and optimal slot utilization
+ Participate in all activities of care as related to outpatient palliative care.
+ Participate in the development and implementation of continuous quality improvement programs.
+ Participate in committee activities.
+ Other related duties as assigned/requested
Knowledge,Skills, and Abilities Required
+ Visual and aural acuity to assess patient status.
+ Ability to interpret and apply principles of nursing research utilization and monitor and report on outcomes and trends.
+ Ability to manage and coordinate palliative care outpatient palliative and supportive services.
+ Work within a collaborative agreement/protocols with a physician and interdisciplinary health care team.
+ Communication and interpersonal skills to work effectively with a wide variety of individuals.
+ Ability to use basic data entry and retrieval systems.
+ Analytic ability to collect and organize information, identify problems and trends, formulate and implement corrective plans or programs, and teach others.

Compensation Statement
Mount Sinai Health System (MSHS) provides a salary range to comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $68.4874 - $68.4874 Hourly. Actual salaries depend on a variety of factors, including experience, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
