Plans, implements and organizes social work resources and evaluates and provides treatment regarding mental health issues for patients; linkage to resources in the community.

Qualifications:

In addition to bringing humankindness to the workplace each day, qualified candidates will need the following:

Master of Social Work (MSW)

6 months home care/hospice experience preferred.

Two years post Master's experience in health care or related field preferred.

Licensed Clinical Social Worker (LCSW) preferred.

Current Driver's license

CPR

Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10 lbs. constantly

This PRN home health Social Worker MSW covers our as-needed field calls. We are interested in candidates with geriatric counselling and hospice experience. The shifts are varied Monday through Friday as needed. We are preparing to cover and increased census.
